This is a dental HMO (DeltaCare® USA) option. As with any HMO, you must select a primary care practice from a network of contracted dental offices and receive all of your care from that practice in order to receive a benefit. The primary care practice will work with the plan when specialty care referrals are required.There are no out-of-network benefits. Copayments under this plan are outlined in the schedule of benefits.

Your out-of-pocket expenses and premiums are often lower with this type of program. This is a comprehensive plan for individuals who are concerned with costs and are willing to choose a dentist from a smaller network.
